Quick note: This one is all over the place.

Crestwood

1) Caer Bronach after you claim it in a small room off the main courtyard next to where they place the Craft Weapons bench. Sitting on a desk on the right hand side of the room.

2) Old Market Road in The Hidden Cave right outside The Village Of Crestwood. Look for the broken down cart on the road and two kitty statues across from it. The Mosaic piece is in the very back on a wooden crate.

3) Flooded Caves in the room across the hallway from the rift room. Laying on what looks like a dining room table in the middle section of the room.

4) Flooded Caves in the bash room found next to the rift room in that same hallway. Down the ladder laying on the right hand table next to the casket.

5) Flooded Caves go out the back of that room I mentioned with the dining room table. Go across the hallway to another room with stairs and then a ladder. Go up the ladder you will find the mosaic piece on a wooden table.

The Fallow Mire

6) Behind the Dwarf Locked room at the castle gate. On the right hand side on the floor just inside the door.

The Forbidden Oasis

7) Near The Oasis Camp in the pond on the right hand side is a mine. Go into that mine and bash through two doors keep going straight. Go up the ladder and straight ahead leaning on the left side wall of the tunnel is the piece.

The Western Approach

8) The Still Ruins (The Lab) in the right hand Dwarf Locked room. On the floor left hand side next to the stone blocks.

9) Coracavus (The Prison) in the Dwarf Locked room laying on a desk.

The Lost Temple Of Dirthamen

10) Behind the magic bash door on the floor next to the loot chest.

11) In the main treasure room after defeating the The Highest One.

The Cradle Of Sulevin

To get this area opened to begin with you have to go to Emprise du Lion and head out past Valeska's Watch past the old broken tower and you will find a random dead elf with a note laying on a cliff. After that you will find Rumors Of The Sulevin Blade on The War Table to trigger.

12) Second Courtyard from the big hole. Small room found on the left hand side in the back. Mosaic piece laying on the floor.
